No it doesnt I have one from my gt grandma and it just states her married name and that she was the widow of.... |

only if that was the name she was registered in. usually a married woman is registered in her married name but the maiden name is never included (other than on marriage cert) http://www.dixons.clara*.co.uk/Certificates/indexbd.htm (remove star) this site will tell you what info is on various certificates. |

It depends on the date.... They do nowadays. I don't know when they started to include maiden name but I'm looking at one which does from 1974. |

not all modern death certificates have a woman's maiden name! My grandmother died last Septembre and her maiden name is definatly not on her death certificate as I've seen the original (my dad registered the death). Also have one from 1987 for my great grandmother and her maiden name is not listed either. |

That's all very strange Lunar. I'm looking at another, which includes maiden name from 1998 and I know I had to give mum's name, when I registered her death in February this year. Is there an English / Welsh difference do you think? Gwyn |

I don't know when it changed but my aunt's death cert from 1975 and mum's from 1993 (both English) are both in portrait format rather than landscape like the old ones and list name and surname in section 2, then next to that section 3 is headed 'sex' and section 4 'Maiden name of woman who has married' |

I registered the death of my sister only last year and was asked for the name of not only her husband but her ex husband too! He told me that I could put as many ex hubbys as I wished! When they see me off, they best have a freshly loaded pen lol Three times married and got my eye on the 4th...lol Gerri (always the bride, never the bridesmaid) |

I think the maiden name on the cert was introduced in Eng/Wales at the same date as the exact date of birth became a requirement.....Q3 1969. Merry |
