Hi I dont belive it does, but know it has date of birth,or date known to be true to the informant,,, if you know her date of birth>,and full name, at death >year of birth,>,and death .>.,you can check this on 1837 ,you cant see any details but it will tell you if there is one to match your criteria,,,, reference to England and Wales ,BDM kay, |

I've just found it on Ancestrys death index ( while looking for something else )........I'd nearly given up.....All the detail are close but are they close enough for me to take a chance !!! esta |

I've just looked at my pile of death certs (all Eng/Wales). I have one from 1982 and there is certainly a box for ''Maiden surname of woman'' on it. Doesn't mean it will be filled in (depending on who did the reg) but at least the registrar will have asked the question. Sarah |

Thanks Sarah I think I will just have to take a chance. If it is my lady I'mm 99% certain she died while in hospital and I'm assuming this could mean that someone at the hospital registered the death. Which is why her maiden name is the only thing I know for certain........if you see what I mean ! Esta |

Well my NHS records still have my maiden name on the front as well as current name, so lets keep our fingers crossed! Sarah |

If it is your local registry office you are ordering from and not online then you can ask them if maiden name is on it and that it's the correct one before you actually order to save you getting the wrong cert. My local office did it for me on a marriage cert i was unsure of wife's maiden name and they checked hope this helps Martin |

Hi Esta, I have a death cert from 1977 and that did have the ladys maiden surname on it so i would say it will do. Claire x |
