What would 'inst. serv' mean on 1881 census

There is a table here of the classifications of occupations http://privatewww.essex.*ac.uk/~matthew/work/occlass.htm 064+ Hospital/Instit. Service It will be Institution(al?) Service MB

I think the 'inst. ser' is just something added afterwards by the enumerator when he was totalling things up and doing whatever clerical things an enumerator has to do. Don't think it is anything to do with the nurses themselves. Kath. x

Address is the Bristol Royal Infirmary transcribed as British and individuals profession described as nurse. But what is inst. serv? Tried google and got anything from instant service to instruction service.
